- Medication monitoring to ensure prescribed treatments are administered properly and consistently, including, healthcare appointments and supervision.
- Permanency and re-entry, with the goal of family reunification, when safe and suitable.
- Supporting alternative stable and permanent long-term placements, such as kinship care or adoption, when reunification is not sustainable.
- Preparing for independent living through life skills training, work experience, and access to resources. Empowering them to become self-sufficient, responsible adults.
- Long-term success through education, training, mentors, positive and other caring relationships with staff, peers, and family members.
